The ideal candidate will hold a Minnesota PT license, have at least 1 year of SNF experience, and demonstrate strong communication, teamwork, and compassionate, patient-centered care.

Qualifications

• Minimum 1 year PT experience; SNF setting preferred.

Responsibilities

• Conduct initial evaluations and assessments to determine patients' physical needs and develop individualized treatment plans.
• Administer therapeutic exercises and activities to improve strength, mobility, and range of motion.
• Utilize modalities (ultrasound, electrical stimulation, heat/cold) to relieve pain and promote healing.
• Educate patients and families on exercises to continue at home for ongoing improvement.
• Monitor progress and adjust treatment plans accordingly.
• Collaborate with doctors and occupational therapists for comprehensive care.
• Maintain accurate documentation of patient progress and treatment plans.

Overview:

We are seeking a dedicated and experienced Physical Therapist to join a team at a skilled nursing facility (SNF) in Virginia, Minnesota. As a Physical Therapist, you will work with patients to help them regain movement and manage pain caused by injuries, illnesses, or surgeries. This is a full-time position with a competitive hourly rate of up to $50 per hour.

Key Responsibilities:

• Conduct initial evaluations and assessments to determine patients' physical needs and develop individualized treatment plans
• Administer therapeutic exercises and activities to improve patients' strength, mobility, and range of motion
• Utilize modalities such as ultrasound, electrical stimulation, and heat/cold therapy to relieve pain and promote healing
• Educate patients and their families on exercises and techniques to continue at home for ongoing improvement
• Monitor patients' progress and adjust treatment plans accordingly

Qualifications:

• Valid Physical Therapist license in the state of Minnesota
