Job summary

Job description

(skilled nursing facility) located in Mapplewood, Minnesota that is seeking a skilled and compassionate Physical Therapist to join their team on a full-time basis. As a Physical Therapist in this role, you will have the opportunity to work with a diverse patient population and make a positive impact on their lives.

Responsibilities:
  • Evaluate patients' physical abilities and develop individualized treatment plans to improve mobility, reduce pain, and increase strength and flexibility
  • Utilize a variety of techniques and modalities, including therapeutic exercises, manual therapy, and equipment, to help patients achieve their treatment goals
  • Monitor and document patients' progress, adjusting treatment plans as needed
  • Educate patients and their families on exercises and techniques to continue their progress outside of therapy sessions
  • Collaborate with other healthcare professionals, such as physicians and occupational therapists, to provide comprehensive care for patients
  • Maintain accurate and up-to-date patient records and communicate with insurance companies as needed
  • Participate in team meetings and continuing education opportunities to stay current on best practices and advancements in the field of physical therapy
Qualifications:
  • Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
  • Current state license to practice as a Physical Therapist
  • Minimum of 1-2 years of experience in a clinical setting, preferably in a SNF or similar environment
  • Strong communication and interpersonal skills to effectively work with patients, families, and other healthcare professionals
  • Ability to create and implement effective treatment plans and adapt them as needed
  • Knowledge of relevant laws, regulations, and best practices in the field of physical therapy
  • Willingness to work with a diverse patient population and provide compassionate care to individuals of all backgrounds
Contract Details:
