Job summary

We are seeking a dedicated and experienced Physical Therapist to join a skilled nursing facility in Virginia, Minnesota. This full-time position offers a competitive hourly rate of up to $50 per hour and the opportunity to help patients regain movement and reduce pain through evidence-based therapies.

The ideal candidate will hold a Minnesota PT license, have at least 1 year of SNF experience, and demonstrate strong communication, teamwork, and compassionate, patient-centered care.

Qualifications

• Must hold Minnesota Physical Therapist license.
• Minimum 1 year PT experience; SNF setting preferred.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct initial evaluations and assessments to determine patients' physical needs and develop individualized treatment plans.
• Administer therapeutic exercises and activities to improve strength, mobility, and range of motion.
• Utilize modalities (ultrasound, electrical stimulation, heat/cold) to relieve pain and promote healing.
• Educate patients and families on exercises to continue at home for ongoing improvement.
• Monitor progress and adjust treatment plans accordingly.
• Collaborate with doctors and occupational therapists for comprehensive care.
• Maintain accurate documentation of patient progress and treatment plans.
