Qualifications

• BLS certification within 30 days (AHA).
• Licensed Physical Therapist (PT).
• Bachelor’s Degree in Physical Therapy is required.

Responsibilities

• Demonstrates safe patient transfers and proper body mechanics.
• Designs and updates a plan of care with goals and outcomes.
• Educates patients, families, and staff using effective teaching methods.
• Documents care and outcomes in the EMR per policy.
• Communicates progress with referring providers and care team.
