Qualifications

Responsibilities

• Perform patient transfers with assistive devices as needed.
• Apply safe body mechanics to prevent injury to self and patients.
• Design and adjust a plan of care including goals, treatment, outcomes, and discharge plan.
• Educate patients, families, and caregivers using effective teaching methods.
• Document findings and progress per policy; communicate with referring providers.
