Job summary

Sherwood Oaks Post Acute (Healthcare) in Thousand Oaks, CA is seeking a part-time Activities Assistant to support the Activities Director in delivering a creative and engaging program that meets residents’ needs under current standards. You will plan and conduct activities, coordinate calendars, assist with assessments and care plans, arrange transportation, and help maintain records.

A high school diploma is required, with 1 year LTC experience preferred.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Responsibilities

• Plan and conduct individual, small and large group activities.
• Assist in providing communication between employees, residents, families, and the public to ensure needs are met.
• Develop monthly activity calendar and maintain attendance records.
• Assist in developing action plans to correct quality deficiencies as requested by Activities Director.
• Assist in discharge planning and development of activity care plans.
• Arrange transportation for residents for outings, appointments, or discharge.
• Develop monthly activity schedule including outings, group activities, and in-room activities for bed-bound residents.
• Encourage resident participation in self-initiated activities and provide needed materials.
