The role involves planning and conducting activities, coordinating with staff and families, and maintaining calendars and records to ensure quality care and engagement for residents.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long-term care facility.
• Ability to read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Responsibilities

• Plan and conduct individual, small and large group activities.
• Assist in communication between staff, residents, families, and the public to meet needs.
• Develop monthly activity calendar and maintain attendance records.
• Assist in discharge planning and activity care plans.
• Arrange transportation for residents as needed including outings and appointments.
• Develop a monthly activity schedule including outings and in-room activities.
• Provide reading materials in Braille or audio books as appropriate.
• Keep the Activity Department clean, orderly and secure.
