Job summary

UT Southwestern Medical Center in Dallas, TX, seeks a licensed clinical social worker for the Palliative Care team. The role provides psychosocial assessment, counseling, and support to patients and families navigating complex care decisions.

Required: Master's in Social Work and state licensure; LCSW preferred or planned within one year. Three years of direct clinical counseling is preferred. This is a full-time, day-shift position with interdisciplinary collaboration.

Qualifications

• Master's in Social Work from an accredited program (CSWE).
• Applicable state license required; LCSW preferred or must obtain within 1 year of hire.
• 3 years of direct clinical counseling experience preferred.
• Advanced Hospice and Palliative Care certification preferred.

Responsibilities

• Liaises for transitional care planning between patient, caregivers and care teams.
• Attends and contributes to interdisciplinary team meetings and care planning.
• Provides independent psychosocial assessment and direct counseling to individuals, couples, and families.
• Provides grief, loss and bereavement support to patients and families.
• Facilitates advance care planning and psychoeducation for staff and families.
• Maintains documentation of social work activities.
• Performs other duties as assigned.
