Responsibilities

• Participates in multidisciplinary rounds and discharge planning including transportation and post-acute placements.
• Utilizes crisis intervention and problem-solving to meet patient and family needs.
• Maintains familiarity with community resources to aid patient outcomes.
• Maintains documentation meeting department standards.
• Reports situations to governmental agencies as required by policy.
• Collaborates with health care team and educates staff, patients, and families.
• Tracks patient placement after discharge on a monthly basis.
• Identifies opportunities for professional growth and participates in PI activities.
