Job summary

Community Healthcare of Texas is seeking a Social Worker in Dallas, TX. This full-time role provides therapeutic and emotional support to patients and families, and helps coordinate end-of-life care with the care team.

Responsibilities include assessing psychosocial needs, counseling, Care Plan implementation, and documenting visits while facilitating referrals to community resources. A Master’s in Social Work and reliable transportation are required.

Qualifications

• Requires MSW
• Proficient computer skills required
• Reliable transportation and valid driver’s license

Responsibilities

• Assess the psychosocial needs of patient and family members
• Provide therapeutic counseling and emotional support to patients and family; educate on end-of-life issues
• Contribute to and execute the Plan of Care with the patient and family
• Record daily documentation of visits and contacts per policy
• Provide referrals to community resources
• Perform other duties as assigned
