Catholic Charities Jacksonville Regional Office is seeking a full-time Emergency Financial Assistance Outreach Coordinator. This in-office role runs 40 hours weekly, Monday to Friday, 8 am to 4:30 pm, in Jacksonville, FL.

Qualifications

• Bachelor's degree in social work, public health, human services, or related field.
• Minimum of two years in outreach, case management, healthcare, or community engagement.
• Experience with individuals living with HIV/AIDS or in healthcare/housing settings preferred.

Responsibilities

• Coordinate outreach, community engagement, and referrals for the HOPWA program.
• Develop relationships with hospitals, clinics, and community partners to increase referrals.
• Track outreach activities and reporting for program evaluation and compliance.
• Ensure timely follow-up and continuity of care for clients.
