Catholic Charities Jacksonville Regional Office is seeking a full-time, in-office Emergency Financial Assistance Case Manager in Jacksonville, FL. The role supports clients through intake, eligibility, and coordinated emergency assistance, aiming to stabilize households and prevent homelessness.

The position requires a Bachelor’s degree in a related field and 2–4 years of case management experience, with a focus on compassionate service and accurate documentation.

Qualifications

• Bachelor’s degree in social work, human services, sociology, psychology, or related field required.
• 2–4 years of case management or direct service experience in nonprofit or social services setting.
• Experience with emergency assistance, housing stability, or low-income populations preferred.

Responsibilities

• Conduct client intake, screening, and needs assessments to determine eligibility for emergency financial assistance programs.
• Develop individualized service plans focused on stabilization and prevention of homelessness.
• Determine eligibility and process financial assistance requests per guidelines and policies.
• Coordinate payment of approved assistance to vendors in collaboration with Finance staff.
• Maintain accurate client records, documentation, and case notes in required databases and files.
• Ensure compliance with grant, contractual, regulatory, and agency requirements.
• Provide referrals to housing, food assistance, employment, benefits, and supportive services.
• Educate clients on budgeting and financial literacy; refer to community resources as appropriate.
• Monitor client progress and follow up on outcomes and ongoing needs.
• Participate in case reviews, team meetings, and supervision; prepare reports for monitoring and evaluation.
• Engage in outreach activities and community collaboration related to emergency assistance services.
• Maintain professional boundaries and ethical standards in all client interactions.
