Clear Destination Inc. is seeking a Case Manager I to work with families and individuals experiencing homelessness or at risk. You will develop care plans, coordinate services, monitor progress, and advocate for clients while maintaining detailed records.

The role emphasizes outreach, crisis intervention, and linkage to housing and mainstream services, with collaboration across the organization and community partners.

Qualifications

• High School Diploma or GED required; bachelor's degree preferred.
• Basic Life Support certification within 30 days of employment.
• Colorado driver’s license valid within 30 days of hire (except SSHC).

Responsibilities

• Manage a caseload of clients with complex needs and ensure care plans are followed.
• Coordinate services, referrals, and housing resources with partners.
• Maintain client documentation and data in the EHR system.
• Provide crisis intervention and advocacy to residents and families.
• Engage in community collaboration to address barriers and improve outcomes.
