Job summary

Ahava Therapy Services in Greeneville, TN is seeking an Occupational Therapist (OT) to join a collaborative in-house rehab team. You will assess residents, develop individualized treatment plans, and monitor progress to maximize function and independence.

Responsibilities include conducting functional assessments, documenting evaluations and progress notes, supervising therapy assistants, educating residents and families, and coordinating with nursing staff and care teams to ensure smooth

Qualifications

• OT degree (Bachelor’s or Master’s) and active OT license
• BLS/CPR certification either current or during orientation
• Strong clinical reasoning, communication, and documentation skills

Responsibilities

• Review clinical information and evaluate its impact on the assessment and treatment process.
• Conduct comprehensive functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable functional goals.
• Document evaluations, daily treatments, progress notes, and discharge summaries accurately and promptly.
• Revise treatment plans based on patient performance and progress.
• Supervise and guide therapy assistants, ensuring proper treatment techniques and the delivery of care.
• Educate residents, families, and caregivers about exercises and therapeutic procedures to support carryover.
• Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional outcomes.
• Assist with discharge planning and maintain knowledge of community resources.
• Participate in care conferences, meetings, and quality improvement initiatives.
• Maintain clinical competence through continuing education opportunities.
• Adhere to all regulatory, professional, and facility standards, including HIPAA and payer guidelines.

• Maintain expected productivity levels and demonstrate excellent teamwork and communication skills.

Qualifications

• Bachelor’s or Master’s Degree in Occupational Therapy
• Active OT license in the state of practice
• BLS/CPR certification (current or obtained during orientation)
• Strong clinical reasoning, communication, and documentation skills
• Ability to collaborate effectively within an interdisciplinary team
