Job summary

LiveWell Homecare Agency is seeking a Licensed Occupational Therapist to provide rehabilitative care in a skilled nursing facility. You will help patients improve strength, balance, and mobility and document progress.

You will collaborate with the healthcare team, educate patients and families on exercises and equipment, and maintain a safe, well-organized treatment area while adhering to safety standards.

Qualifications

• Active state licensure as an Occupational Therapist.
• Minimum of 1 year of relevant experience.
• Associate's degree in Occupational Therapy Assistant program.
• Excellent communication, attention to detail, and ability to work independently or in a team.

Responsibilities

• Provide therapy services to improve patients' strength, balance, and mobility.
• Observe, document, and track patient progress.
• Instruct patients and families on exercise techniques and equipment use.
• Collaborate with healthcare team members to coordinate care.
