Job summary

LiveWell Homecare Agency, LLC is seeking a Licensed Occupational Therapist to provide rehabilitative care in a skilled nursing facility. This full-time role focuses on improving mobility, strength, and balance through individualized exercises and activities to support patient recovery.

You will deliver therapeutic services, educate patients and families, coordinate with the health care team, maintain safe treatment areas, and participate in ongoing training to enhance skills.

Qualifications

• Active state license as an Occupational Therapist is required.
• Minimum of 1 year of relevant experience.
• Strong communication, attention to detail, and teamwork skills.
• Knowledge of physical therapy principles.
• Physical stamina for assisting patients and performing tasks.
