Job summary

LiveWell Homecare Agency, LLC is seeking a qualified Occupational Therapist to assess, plan and participate in rehabilitative programs aimed at restoring daily activities, mobility and cognition for patients. You will collaborate with physicians to tailor treatment plans and discharge strategies.

You will educate patients and families, document progress, and ensure compliance with departmental protocols while maintaining patient confidentiality and a respectful team environment.

Qualifications

• Must hold an accredited occupational therapy degree (Bachelor’s or Master).
• OT licensure required on hire; BLS certification preferred or required upon hire.
• Strong communication, customer service orientation, teamwork and basic computer skills.

Responsibilities

• Collaborate with physicians to plan and evaluate treatment plans and discharge strategies.
• Implement therapeutic interventions and adjust plans based on patient progress.
• Educate patients and families, provide discharge instructions and refer to resources.
• Document assessments, sessions and discharge planning per departmental protocols.
• Maintain confidentiality and ethical standards; foster a team-oriented environment.
• Attend staff meetings, ongoing education and mandatory training for growth and compliance.
