Job summary

Creative Solutions in Healthcare, Inc. is seeking a dedicated Full-Time Occupational Therapist (OT) to join our in-house rehab team in Texas.

You will evaluate residents’ daily occupations and develop individualized plans to improve independence, safety, and quality of life through meaningful activities and functional outcomes. The role requires an active Texas OT license, SNF or long-term care experience preferred, and strong collaboration with an interdisciplinary team.

Qualifications

• Active and valid Texas Occupational Therapy license.
• Experience in SNF or long-term care settings preferred.
• Strong communication and clinical decision-making abilities.

Responsibilities

• Consult with patients to evaluate their condition and daily impact.
• Develop individualized treatment plans to maximize ADLs and IADLs.
• Assess home and living environments and recommend modifications.
• Recommend adaptive equipment and provide training to support independence.
• Ensure treatments align with the care plan and comply with regulations.
• Collaborate with the interdisciplinary team to deliver high-quality care.
• Complete documentation timely and in accordance with policies.
