Job summary

CVS Health is seeking a Case Management Coordinator in Ohio to support the OhioRISE program. This full-time, field-based telework role requires travel within Northeast Ohio up to 50% to member homes and locations.

Strong coordination, education, and care management skills are essential to improve healthcare outcomes for children and families within Medicaid services. Qualified candidates have a Bachelor's degree or non-licensed master level clinician, 2+ years in behavioral health or child

Qualifications

• Bachelor’s degree or non-licensed master level clinician required
• 2+ years of experience in behavioral health, social services, or human services
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint)
• 2+ years in children's mental health, child welfare, or related field
• 2+ years in family systems, case management, or related area
• Willing to travel up to 50% and may travel to New Albany office

Responsibilities

• Coordinate and implement care plan activities and monitor progress
• Evaluate member needs and eligibility and coordinate appropriate services
• Consult with case managers, supervisors and medical directors to overcome barriers
• Provide coaching and support to empower members to participate in healthcare decisions
• Serve as single point of contact for members and address gaps in care
