City View Post Acute in San Francisco, CA, is seeking a detail-oriented MDS Nurse (LVN) for a full-time role. You will coordinate MDS assessments, support care planning, and help ensure compliance with CMS and state requirements while contributing to quality resident outcomes.

Qualifications

• Must hold an active California LVN license.
• Experience coordinating MDS assessments and care planning.
• Knowledge of CMS regulations and state requirements.

Responsibilities

• Complete MDS 3.0 assessments and CAAs within required timelines.
• Maintain complete and accurate clinical documentation to support care planning and reimbursement.
• Ensure compliance with CMS regulations, California requirements, and facility policies.
• Collaborate with the interdisciplinary team to develop and update individualized resident care plans.
• Monitor resident progress and communicate changes to the care team.
• Participate in resident care conferences and quality improvement initiatives.
• Ensure MDS submissions meet Medicare/Medi-Cal documentation and reimbursement requirements.
• Assist with audits, surveys, and regulatory reviews by providing accurate documentation and reports.
• Stay current with changes to MDS requirements, regulations, and nursing documentation standards.
• Educate and guide staff on documentation practices and MDS requirements.
• Communicate professionally with residents, families, and team members regarding care planning.
