TriHealth's Good Samaritan Hospital in Cincinnati is seeking a Social Worker to assess psychosocial needs and develop a comprehensive discharge plan within Maternity Inpatient Services. You will collaborate with a multidisciplinary team to promote patient well-being and smooth transitions.

The ideal candidate has a MSW, licensure as a Social Worker, and up to one year of clinical healthcare or community social work experience, plus strong counseling and community resource skills.

Formación

• Master's Degree in Social Science or Social Work required
• Up to 1 year clinical healthcare or community social work experience required
• Computer literate
• Experience in grief counseling, substance use intervention, and abuse/violence support preferred
• Rquirement: Licensed Social Worker in good standing

Responsabilidades

• Assess psychosocial needs and develop a plan for interventions and complex discharges
• Collaborate with multidisciplinary team to ensure effective, appropriate care and smooth transitions between levels of care
• Provide education and support to promote social work practice within the hospital
• Serve as resource and preceptor for new staff and provide on-call services as needed
• Respond to referrals and assess psychosocial and discharge needs using medical records and team input
