Qualifications

• MSW or equivalent degree required.
• Up to 1 year clinical healthcare/community social work experience required.
• Licensed Social Worker required.
• Experience with grief, bereavement counseling, and behavioral health interventions preferred.

Responsibilities

• Assess psychosocial needs and develop a care plan for discharge.
• Collaborate with multidisciplinary team to coordinate care and transitions.
• Provide education and support to patients and families.
• Serve as a resource and preceptor for new staff as needed.
• Respond to referrals and assess complex discharge needs.
