Job summary

The Christ Hospital is seeking a social worker to assess psychosocial needs and coordinate discharge planning for patients and families. You will provide counseling, advocacy, and linkage to community resources as part of a multidisciplinary team focused on safe, seamless transitions of care.

Required MSW with LSW/LISW licensure, and a minimum of 2 years of clinical experience in hospital, long-term care, or hospice settings. Ongoing licensure is mandatory to practice in Ohio.

Qualifications

• Master's degree in social work (MSW) required.
• LSW/LISW required.
• 2 years clinical experience in hospital, long-term care or hospice setting preferred.
• Assure ongoing licensure through the State of Ohio Counselor, Social Worker, and Marriage and Family Therapist Board.

Responsibilities

• Assess psychosocial needs and discharge planning for patients and families.
• Provide psychosocial counseling and advocacy.
• Coordinate discharge planning with interdisciplinary team to support readmission prevention.
• Educate patients and families about health care resources and community supports.
