Qualifications

• MSW required from an accredited School of Social Work.
• Licensed by the State of Ohio Social Work Board (LSW, LISW, or LISW-S). For UC Health Psychiatric Services, NCI-APS Certification must be obtained within 90 days of hire.
• 1–2 years of related experience preferred: 3–5 years preferred.

Responsabilités

• Respond to referrals from the interdisciplinary team.
• Complete psychosocial assessments and develop a plan of care to promote continuity of care.
• Provide education, consultation, and serve as liaison to the healthcare team on psychosocial aspects of care.
• Engage patients and families into a helping process and assess psychosocial/mental health needs.
• Develop and implement comprehensive intervention plans in collaboration with patients, families, and the healthcare team.
• Provide individual/family counseling and social work interventions for crises, illness adjustments, and barriers to care.
• Advocate, mediate and negotiate to maintain or enhance patient health and social supports; facilitate discharge planning and referrals to community resources.
• Provide education to patients and families, assist with advance directives, and contribute to quality and safety initiatives.

Formation

Master of Social Work (MSW)

Ohio Social Work License (LSW/LISW/LISW-S)

NCI-APS Certification (within 90 days)
