Job summary

Signature HealthCARE, LLC in Terre Haute, IN is seeking an experienced LPN/LVN to provide direct nursing care to residents, including medication administration, wound care, and skilled procedures. You will supervise nursing assistants and coordinate care with the interdisciplinary team in a 176-bed facility.

Ideal candidates have a current LPN/LVN license, at least 1 year of clinical nursing experience (long-term care preferred), and familiarity with EHR systems.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of Federal and State long-term care regulations.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including meds, wound care, and procedures.
• Supervise and direct nursing assistants and medication aides during the shift.
• Monitor and observe residents’ status and notify physician of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation.
• Coordinate admissions, transfers and discharges.
• Participate in QA activities and regulatory surveys.
• Perform other duties as assigned.
