Job summary

Mountain View Hospital is seeking a Licensed Clinical Social Worker (LCSW) to support clinical integration, improve patient outcomes, and enhance satisfaction across MVH Primary Care Practices.

The LCSW will assess, intervene, and advocate for patients with varying mental health needs, coordinating with PCPs and other providers in a flexible role that emphasizes community resources and compassionate care.

Qualifications

• Licensed Clinical Social Worker certification or license required.
• Two to five years clinical experience preferred; minimum 6 months required.
• Proficiency with Word, PowerPoint, Excel and EMR systems.
• Excellent written and oral communication.
• Strong interpersonal skills and compassion for patients and families.
• Ability to maintain confidentiality and handle sensitive information.

Responsibilities

• Completes initial screening of patients referred from primary care providers to determine disposition and needs.
• Employs a wide range of interventions, such as care coordination, risk screening and behavior change, educational information, referrals, and safety planning.
• Advocates for patients to gain access to services and resources.
• Coordinates and supervises patient care with outside practices as needed.
• Maintains patient confidentiality and data in EMR systems; documents outcomes.
• Engages with patients and families to develop age-appropriate care plans and education.
