Providence St. Joseph Hospital Orange, CA is seeking a Part-time Care Manager LCSW to join our inpatient team. This 8-hour daytime shift role supports at-risk patients with comprehensive, hospital-based care management.

Responsibilities include assessment, planning, care coordination, discharge planning, documentation, regulatory compliance and patient advocacy to remove barriers to resources and care. Minimum qualifications: MSW (CSWE-accredited), California LCSW license, 2 years healthcare

Qualifications

• Master's Degree in Social Work from an Accredited CSWE program.
• California Clinical Social Worker License upon hire.
• 2 years social work experience in healthcare.
• 1 year in acute care setting or TIPs/CM orientation.

Responsibilities

• Assessment and planning for at-risk patients in an acute care setting.
• Coordination of care and discharge planning.
• Documentation of interventions and regulatory compliance.
