• Experience: 6 months required, 2-5 years clinical preferred.

Responsibilities

• Completes initial screening of patients referred from primary care to determine disposition and needs.
• Employs care coordination and case management, risk screening, and safety planning.
• Offers solutions to road blocks while implementing programs.
• Monitors high-risk patients and follows up with clinic staff.
• Ensures deadlines are met during implementation stages.
• Revises care plans based on patient response and clinician direction.
• Maintains patient confidentiality and coordinates care with providers.
