Imperial Health Plan of California, Inc. is seeking a Licensed Clinical Social Worker (LCSW) to join the Case Management team under the supervision of the Manager/Supervisor of Case Management.

This role focuses on assessing emotional and social needs, coordinating with families and caregivers, and connecting members with community services for comprehensive care. You will interview members, assess medical, psychological, emotional, and social needs, and establish action plans with the member

Qualifikationen

• Bi-Lingual English/Spanish required.
• California Licensed Clinical Social Worker license required.
• Bachelor’s or Master’s degree from an accredited school.
• Experience in multicultural health care delivery.

Aufgaben

• Counsel on substance abuse and rehab strategies.
• Coordinate social services and facilitate care transitions.
• Collaborate with ACM/ICM and coordinators to coordinate medical, behavioral, social, and other services including LTSS.
• Develop and maintain complex treatment and progress reports and documents.
• Advise members of available community resources and act as liaison for member/families.
