Qualifications

• Master’s degree in Social Work is required.
• Florida license as a Licensed Clinical Social Worker (LCSW) per Florida Statutes chapter 491.
• At least three years of direct clinical experience.
• For Home Health, a valid Florida driver’s license is required.

Responsibilities

• Collaborates with the clinical team to identify patients who will benefit from care coordination.
• Facilitates comprehensive discharge planning and follow-up care.
• Engages patients and families to support self-management and adherence to care plans.
• Uses IT resources to support registry reporting, track patient populations and outcomes.
• Promotes patient safety and regulatory compliance in daily tasks.
• Provides coverage and supervision as needed in the department.
