Job summary

Children’s Network of Hillsborough LLC is seeking a Family Team Facilitator to provide direction, support, and technical assistance to sub-contracted providers. You will monitor placement stability, coordinate meetings, and help develop collaborative plans to safeguard children and promote permanency within families.

The role involves staffings, bed hold and funding authorizations, and participation in Family Team Conferences to drive safe outcomes in a complex lead agency system.

Qualifications

• Bachelor's degree in social work, counseling, human services, psychology, or related field.
• Minimum two years of child welfare experience with certification preferred.
• Ability to track, review, and report on placement stability and related metrics.
• Strong leadership and communication skills with the ability to collaborate across teams.

Responsibilities

• Track, review and report on placement stability and placement-related metrics.
• Facilitate meetings with residential group homes and foster parents to address systemic issues.
• Participate in clinical staffing and MDTs to address high-end placements and step-down planning.
• Coordinate bed hold requests and related funding authorizations.
• Lead Family Team Conferences to promote safe and healthy outcomes for children.
• Monitor academic performance and educational success of children in care.
• Assist in re-licensing of residential group homes to ensure compliance with laws and standards.
