Job summary

CENTERS FOR FAMILY DEVELOPMENT INCORPORATED seeks an ICCW Coordinator to provide intensive Wraparound services for children, youth, and families with complex behavioral health needs in Detroit, MI.

The role centers on leading the Child and Family Team, coordinating services across behavioral health, education, child welfare, and juvenile justice while ensuring culturally responsive, trauma-informed support.

Qualifications

• Bachelor’s degree required with experience in family-centered, strength-based practice.
• Experience coordinating across child-serving systems and with Wraparound models preferred.
• Completion of ICCW/Wraparound training and ongoing certification preferred.

Responsibilities

• Facilitate all phases of the Wraparound process and lead Child and Family Team meetings.
• Provide intensive care coordination, case management, and advocacy for families.
• Develop and monitor individualized Plans of Care across multiple systems.
• Ensure fidelity to the ICCW model and maintain required documentation.
• Maintain an active caseload of 8–10 families, up to 12 during transitions.
