Judson Center in Wayne County, MI is seeking an ICCW Facilitator to provide intensive care coordination with wraparound services. Qualifications

• Bachelor’s degree in human services or related field.
• Valid Michigan Driver’s License and Vehicle Insurance
• Must complete MDHHS ICCW trainings for new facilitators in Lansing, MI
• Minimum one year post-degree mental health experience, preferably with severely emotionally disturbed individuals
• Understanding of criminal justice delinquency, neglect/abuse issues, and family systems in social work contexts

Responsabilités

• Facilitate family preservation and support services for ICCW-eligible families via Children’s Special Initiatives
• Conduct strengths and psychosocial assessments
• Develop family service plans with the Child and Family Team
• Provide or arrange outreach crisis intervention and case management services
• Link and advocate with other agencies to ensure access to health and human services
• Provide support to enhance social functioning in home, school, and communities
• Monitor treatment plan implementation and create crisis-support plans with the team
• Attend required trainings and all scheduled team meetings
• Perform other duties as assigned
