Job summary

Association for Individual Development is seeking a dedicated Case Manager QIDP to provide clinical case management for up to 20 individuals with I/DD, coordinating internal and external services to maximize client outcomes and community inclusion.

The role requires a Bachelor's in Human Services and preferred QIDP/QMHP experience. Strong documentation, advocacy, and licensure compliance are essential; hybrid work schedule may be discussed with client needs.

Qualifications

• Bachelor's degree in Human Services required.
• One year experience as QIDP or QMHP preferred.

Responsibilities

• Manage caseload of up to 20 clients and maintain caseload assignments.
• Maintain client case records in electronic system (Cx360) and update annually or as needed.
• Schedule and lead service planning meetings per licensure timelines.
• Coordinate health, behavioral, community day, behavioral health, and employment services for clients.
• Complete referrals and coordinate external services; ensure documentation in client case record.
• Ensure client rights and privacy per DHS, HCBS, and CARF standards; monitor compliance.
