Job summary

Compass Career Solutions, sister company to Dungarvin, is seeking a Housing Case Manager to assess clients, create action plans, and document care with internal and external teams in Washington.

This in-office position requires daily attendance and outreach, with responsibilities including connecting clients to medical, mental health, and social services to enhance independence and life quality.

Qualifications

• Bachelor's Degree in social services or related field preferred with one year of housing/independent living coordination experience, or
• High School Diploma or GED with two years of experience in coordinating supportive housing or independent living services.

Responsibilities

• Conduct assessments and interviews with clients to determine eligibility.
• Develop and update action plans, case notes, and related documents.
• Document care and support plans with clear communication among internal and external teams.
• Provide ongoing life skills development to improve client independence and quality of life.
• Offer targeted interventions when clients are at risk of eviction.
• Connect clients to medical, mental health, and other supportive services.
• Refer clients to community services and coordinate referrals.
• Assist clients with SSI/SSDI, Medicaid, Medicare, SNAP applications and re-determinations.
