Job summary

Abode Services is seeking a Service Coordinator to provide housing services for individuals experiencing homelessness in Alameda County. The role focuses on counseling, referrals to financial, legal, and supportive services, and creating individualized housing plans to achieve stability and permanent housing.

You will work with a collaborative team to connect participants to essential services, coordinate with partners, and document outcomes.

Qualifications

• Bachelor's degree in Psychology, Human Services, Social Work, Sociology, or related field or equivalent field experience.
• 2 years case management experience providing services to homeless or low-income individuals and/or families.
• Driving and transportation of participants when required.

Responsibilities

• Provision of comprehensive housing services to participants, including working with landlords.
• Coordinate with social service partners to refer and provide services.
• Ensure participants receive essential services to improve health and housing outcomes.
• Assist residents in retaining housing and maximizing independence with linkages to community services such as crisis intervention and income support.
• Provide referrals for assessment/treatment in areas like developmental or behavioral health, school readiness, and exposure to substances.
• Create and maintain an Individual service plan and needs assessment for each household.
• Meet with households in person as needed, in the community or at home.
• Attend program meetings with partners to coordinate services and improve quality.
• Maintain client files and documentation.
• Communicate with the team to solve problems and share resources.
• Document services in a timely manner using standard formats.
• Attend staff meetings and other agency functions as needed.
• Other duties as assigned.
