Job summary

PATH seeks a Case Manager for our Villas Marina Del Rey site in Los Angeles, CA. You will conduct intake, develop individualized service plans, and coordinate housing goals, income stabilization, and supportive services for clients experiencing homelessness.

Responsibilities

Conducts intake and needs assessments for clients to develop ISP addressing housing and income goals.
Provides referrals and support to resources to meet ISP goals.
Completes program assessments (e.g. VI-SPDAT).
Evaluates progress with client at 90-day reviews and maintains case files with required documents.
Maintains confidentiality per HIPAA and guidelines.
Ensures documentation standards and timely case notes.
