Job summary

AID Atlanta, Inc. is seeking a skilled social services professional to join its HOPWA/HINT housing program team.

You will provide direct service coordination, case management, and life skills support to people living with HIV/AIDS, helping them access housing and sustain medical care. This role requires a Bachelor's degree in a related field and 1–2 years of experience; you will work with partners, conduct assessments, develop service plans, and document encounters in EMR systems to support

Qualifications

• Bachelor’s degree in social work, psychology, sociology, human services or social services-related field.
• 1-2 years’ experience in social work/case management or similar field.

Responsibilities

• Provide direct service coordination, coaching and life skills support to people living with HIV/AIDS in need of housing assistance.
• Provide site based enhanced and intensive case management services to HOPWA HINT Program members on a walk-in, scheduled, and on-call basis.
• Manage a caseload of up to 30 members.
• Conduct comprehensive initial assessment and low barrier intake for eligible members.
• Assist with housing applications, TBRA subsidy coordination, and rent calculations.
• Provide weekly housing support contact to follow up on treatment and housing goals.
• Coordinate with housing properties to assist members with housing placement.
• Develop ISP/HSP plans and conduct quarterly updates with each member.
• Assist with social service referrals, voucher applications, and transportation needs.
• Educate members on services, policies, and rights; connect to employment and financial services.
• Document encounters in EMR within 24 hours and track outcomes for program reporting.
