Job summary

Riverside Hospice Agencies is seeking a Spiritual Counselor to provide emotional and spiritual support to patients, families, and caregivers as part of the interdisciplinary team. The role supports Riverside Hospice Agencies' mission and care philosophy.

The counselor will assess patients promptly after admission, develop personalized care plans in collaboration with clergy as desired, and ensure documentation and coordination with the care team. Weekend and call rotation may be required.

Qualifications

• Master's degree in pastoral care or theology is required.
• 1 year hospice experience is preferred.
• ACPE Educator Certification is preferred (within 2 years).
• Valid Driver's License is required.

Responsibilities

• Provide assessment and spiritual support for patients and families.
• Collaborate with interdisciplinary team and CHAP standards.
• Develop individualized spiritual care plans in partnership with patients and clergy as desired.
• Document counseling provided in patient records and monitor care plan updates.
• Participate in training and support of team members and coordinate with other spiritual coordinators.
