Job summary

Riverside Health is seeking a Spiritual Counselor to provide emotional and spiritual support to patients, families and caregivers within the hospice program. You will participate as a member of the interdisciplinary team and support the mission of Riverside Hospice Agencies.

The role requires a Masters Degree in Pastoral Care or Theological Studies, hospice experience preferred, and relevant certifications.

Qualifications

• Masters Degree in Pastoral Care or Theological Studies (Required)
• 1 year hospice-related experience (Preferred)
• ACPE Educator Certification within 2 years (Preferred)

Responsibilities

• Provide spiritual/pastoral assessments and develop an individualized plan of care in collaboration with local clergy when desired by the patient and family.
• Communicate patient care needs and changes to the physician and interdisciplinary team, aligning with current standards.
• Document counseling in the patient’s clinical record per RHS standards.
• Assist in plan of care development and update; conduct religious services and support patients/families of different beliefs.
• Supervise Hospice Spiritual Counselors and participate in supervisory activities for annual reviews.
• Participate in team training and support of staff, volunteers, and students.
