Job summary

Riverside Home Health and Hospice – Gloucester, Virginia seeks a Spiritual Counselor to provide emotional and spiritual support to patients and families. The position requires a Master’s degree in Pastoral Care or Theology and at least 1 year of hospice experience, with preferred ACPE Educator Certification.

Qualifications

Responsibilities

• Provide assessment and spiritual support after admission based on patient/family wishes.
• Develop individualized spiritual/pastoral care plans in collaboration with local clergy when desired.
• Participate in interdisciplinary team meetings to discuss quality and outcomes.
• Document counseling and care plans in the patient’s chart.
• Support care coordination and communication with physicians and team members.
• Engage in training and support of team members and other spiritual coordinators.
