Job summary

Riverside Hospice Agencies seeks a Spiritual Counselor to provide emotional and spiritual support to patients, families and caregivers. You will join an interdisciplinary team and help advance the mission of Riverside Hospice Agencies.

You will assess patients promptly after admission, develop individualized care plans, and collaborate with clergy and other professionals to meet patient and family needs, while documenting care and supporting quality outcomes.

Qualifications

• Master’s degree in Pastoral Care or Theology is required.
• American Council of Pastoral Education 1 unit upon hire is required.
• ACPE Educator Certification within 2 years is preferred.
• 1 year hospice experience is preferred.

Responsibilities

• Provides assessment after admission and supports patient/family needs.
• Completes required continuing education classes and stays current in clinical knowledge.
• Participates in interdisciplinary team meetings regarding quality and outcomes.
• Develops a spiritual plan of care with clergy collaboration as desired by patient and family.
• Notifies physician and team of changes in patient needs and outcomes.

Education
