Patriot Healthcare in Myrtle Beach, SC is hiring an RN Case Manager to coordinate hospice services according to physician orders and patient plans of care. You will work within an interdisciplinary team to ensure compassionate, high-quality care aligned with hospice policies and standards.

The role requires SC RN licensure and a nursing degree; hospice experience is preferred. A strong emphasis on documentation, collaboration with IDT, and adherence to regulations is essential for success.

Qualifications

• Nursing degree from an accredited college or university.
• Licensed as a Registered Nurse in the State of South Carolina.
• 1 year of experience as a nurse in a hospital, long-term care facility, or other related health care facility is preferred.
• Hospice experience also preferred.

Responsibilities

• Coordinate, direct, and oversee hospice services according to physician orders and patient plan of treatment.
• Serve as a member of the Interdisciplinary Team (IDT) and attend meetings.
• Follow the Nursing Standards of PH Healthcare and comply with hospice policies and regulations.
• Complete medical forms, reports, evaluations, and charting as necessary.
• Supervise Hospice Aide care and CNA schedules.
• Report to the Director of Clinical Services and participate in quality assurance programs.
• Maintain liaison with facility nursing staff and plan of care for hospice patients.
