Patriot Healthcare in Sumter, SC is seeking an experienced Registered Nurse Case Manager to join our compassionate hospice team. You will coordinate and deliver patient-centered care in collaboration with the Interdisciplinary Team, ensuring comfort, dignity, and quality of life for patients and families.

• Minimum 1 year of nursing experience; hospice experience preferred.

Responsabilidades

• Coordinate and direct hospice services per physician orders and patient plan of treatment.
• Participate in the Interdisciplinary Team (IDT) meetings.
• Document nursing services for billing and compliance.
• Supervise Hospice Aide care and CNA schedules.
• Report occupational exposures as per policy.
