Job summary

Ohio Living is seeking an RN Case Manager - Hospice to plan, coordinate, and deliver quality hospice care in alignment with company standards and regulations. You will assess patients, develop care plans and educate families while collaborating with other providers.

A current Ohio RN license and at least 1 year in acute or medical-surgical nursing are required; hospice experience is preferred. Some driving, weekends and on-call duties may apply.

Qualifications

• Current unencumbered license for the state of Ohio to practice as an RN.
• One year experience in acute care or medical-surgical nursing.
• Hospice experience preferred.
• Proficiency with Windows, Microsoft Office, and the internet; EMR preferred.

Responsibilities

• Plan, coordinate and deliver quality hospice care per standards and regulations.
• Assess patients, develop care plans and oversee Plan of Care implementation.
• Educate patients and families; communicate changes with healthcare providers.
• Maintain accurate records and participate in quality/compliance activities.
• Perform additional duties as assigned.
