OhioHealth at Home in partnership with Compassus is seeking a Hospice Registered Nurse / RN Case Manager to deliver compassionate, in-home hospice care. You’ll collaborate with a care team, educate families, and advocate for comfort, dignity, and peace for patients at home.

Requirements include an active RN license, 2 years of RN experience with hospice preferred, and CPR/BCLS certification. Excellent communication and care-planning skills are essential.

Qualifications

• RN license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Current CPR and BCLS certification.

Responsibilities

• Provide patient- and family-centered hospice care at home.
• Coordinate with care teams to develop and adjust care plans.
• Educate caregivers on home-based care and symptom management.
• Assist patients in spending quality time with loved ones.
