Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to deliver in-home evaluation and treatment for patients, reporting to the Clinical Manager. You will contribute to the plan of treatment, assess disorders of speech, voice, language, hearing and swallowing, and document progress to the supervising physician.

With required ASHA CCC-SLP or CCC-A, a minimum six months of experience, and a valid license and CPR, you’ll join a Humana-affiliated team committed to

Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
• Minimum six months experience as a speech therapist / speech language pathologist
• Current and unrestricted license
• Current CPR certification
• A valid driver’s license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize effects of such disorders
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening
• Observe, record and report changes in the patient’s condition and response to treatment to supervisor/physician
