Qualifications

• Must hold CCC-SLP certification from ASHA and state licensure.
• Minimum six months as a speech-language pathologist or therapist.

Responsibilities

• Evaluate and provide speech-language pathology services in home or facility.
• Develop and review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests relevant to speech therapy.
• Document patient progress and communicate with the supervisor/physician.
• Provide training on alternative communication systems when appropriate.
• Support patients, families and healthcare staff with counseling and instruction.
• Participate in care coordination and discharge planning.
