Job summary

Humana Inc. through CenterWell Home Health seeks a Home Health Speech Language Pathologist to evaluate and treat patients in their homes, focusing on rehabilitation and communication disorders.

You will work under the Clinical Manager to develop and review plans of care, document progress, and collaborate with families and care teams to support patient independence and quality of life.

Qualifications

• Must hold Certificate of Clinical Competence in Speech-Language Pathology (or Audiology) per ASHA.
• Minimum six months of experience as a speech-language pathologist.
• Current CPR certification required.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Evaluate, direct and provide speech-language pathology services in home or facility.
• Develop and review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic interventions.
• Record, report and communicate changes in patient condition to supervisor/physician.
• Instruct patients in alternative communication methods when appropriate.
• Counsel patients, families and healthcare staff; maintain documentation as required.
• Prepare written reports of evaluations, visits, and care plans.
• Participate in care coordination and discharge planning.
