Job summary

CenterWell Home Health, part of Humana Inc., is seeking a Home Health Speech Language Pathologist to deliver rehabilitation-focused care to patients at home or in facilities. You will evaluate communication and swallowing disorders, contribute to plans of treatment, and help patients regain independence.

You will conduct assessments, document progress, educate patients and families, and coordinate with care teams.

Qualifications

• Certification of Clinical Competence in Speech-Language Pathology or Audiology per ASHA is required.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted professional license.
• Current CPR certification.
• Valid driver’s license with reliable transportation and auto insurance.

Responsibilities

• Evaluate and provide speech-language pathology services to patients in home or facility.
• Develop and review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer diagnostic tests and apply therapeutic interventions.
• Record and report changes to supervisor and physician.
• Provide instruction to patients and families in communication strategies.
• Maintain documentation and submit reports per policy.
• Participate in care coordination and discharge planning.
